One monthly fee covers your primary care needs including unlimited in-person and virtual visits, improved access to your physician, and clinically indicated in-office procedures. Through membership you are investing into your health, preventing instead of reacting. Say goodbye to copays.

This is not insurance — we recommend maintaining a basic insurance plan for when care is needed beyond our clinic services.

How enrollment works?

Choose your plan and complete the online enrollment process.
Review and sign the patient agreement.
We’ll reach out to confirm and provide an intake packet along with scheduling your 1st appointment.

Pediatrics

Known Patients

15% off if enrolled in 1st year of opening

Family Plan

Maximum $350/month

Up to 2 adults + 2 pediatric dependents
Additional children: $25/month

Pre-enrollment is open now. Billing and care begin November 16, 2026. SigmaMD collects payment details at enrollment, but your first charge is not until November 16, 2026. NOTE: Credit card payments include a 2.99% processing surcharge. There is no surcharge when you pay by bank transfer (ACH) or debit card.

All memberships are HSA-eligible. Individuals can apply up to $150/month, families up to $300/month in HSA funds. See FAQ for details.

Known Patient Discount

Confirmed known patients/colleagues of Dr. Nguyen from prior practices who sign up in the first 12 months from opening will receive:
15% off membership pricing
(Discount applies while membership remains active; standard pricing resumes if membership is canceled and later restarted.)
